Total Compensation: $54.49–$57.76/hour*

Help patients turn everyday activities into meaningful rehabilitation goals. Travel between home visits, assess mobility and safety, guide therapeutic treatment and teach patients and caregivers how to keep progressing. You’ll coordinate with the wider care team while enjoying the variety and independence of community-based physical therapy.

• 13-week W-2 contract; 1 opening
• Monday-Friday, 8 a.m.-4 p.m.
• 37 guaranteed hours per week; the weekday schedule totals 40 hours.
• Proposed start: October 28, 2026, pending facility confirmation.
• Travel between patient homes is part of the role.

What You Bring

• Vermont physical therapist license by acceptance.
• At least two years of relevant experience.
• Home-health assessment, OASIS and care-coordination skills.

Pay and Benefits

Taxable W-2 hourly pay: $22.82–$26.09.
