Assignment Overview

• Newport, Vermont 05855
• 13-week W-2 contract; 1 opening
• Monday-Friday, 8 a.m.-5 p.m.; holiday coverage as needed.
• 32 guaranteed hours per week; compensation estimate uses a 40-hour planning week.
• Proposed start: October 28, 2026, pending facility confirmation.
• Travel between patient homes is part of the role.

What You Bring

• Active Vermont physical therapist license.
• At least two years of experience.
• OASIS, home-health and hospice experience required.
• Experience traveling between home visits and adapting the daily visit schedule.

Pay and Benefits

Taxable W-2 hourly pay: $20.00–$23.27.
