Your Work

Help patients connect the pieces of their care. This role combines care coordination, education and follow-through as you work with patients, families and the clinical team to make treatment plans easier to understand and navigate.

Coordinate patient care, utilization review, discharge planning and transitions with the clinical team and available community resources.

Requirements

• Active Hawaii RN license and applicable hospital life-support credentials.
• At least two years of acute-care case-management experience.

Pay and Benefits

Taxable W-2 hourly pay: $20.00–$23.05.
