Total Compensation: $53.29–$56.56/hour*

Make a difference one home visit at a time. Provide individualized, goal-oriented nursing care and take the lead as case manager when nursing is the primary discipline. You’ll coordinate with patients, families and the wider care team, helping people receive thoughtful care in familiar surroundings. This is a chance to pair clinical independence with meaningful continuity of care.

Assignment Overview

• Barre, Vermont 05641
• 20-week W-2 contract; 4 openings
• Monday-Friday days, with a weekend rotation approximately every fourth week.
• 36 guaranteed hours per week; compensation estimate uses a 40-hour planning week.
• Proposed start: November 10, 2026, pending facility confirmation.
• Travel between patient homes is part of the role.

What You Bring

• Active Vermont RN license.
• BLS certification.
• At least one year of relevant experience.
• Ability to manage individualized home-care plans and document in Homecare Homebase.

Pay and Benefits

Taxable W-2 hourly pay: $20.00–$23.27.
