Total Compensation: $61.14–$64.48/hour*

Help patients make meaningful progress in the place they call home. Evaluate mobility and home safety, develop individualized treatment plans and work with patients and caregivers on strength, balance and daily function. This home-health and hospice assignment combines independent visits with close collaboration across the care team, and a facility vehicle is available for patient visits.

• 13-week W-2 contract; 1 opening
• Day shifts, 36 hours per week.
• 36 guaranteed hours per week; hourly equivalent uses that weekly basis.
• No call.
• Proposed 2026-10-26 start, pending facility confirmation.
• A facility vehicle is available for patient visits.

What You Bring

• Active Montana physical therapist license and BLS.
• At least two years of relevant experience.
• Valid driver license; ability to travel to patient homes.
• Experience with assessment, treatment planning, patient education and timely clinical documentation.
