• Fort Defiance, Arizona 86504
• 13-week W-2 travel contract; 2 openings
• Monday to Friday, 8:00 a.m. to 5:00 p.m. with one-hour lunch (8-Hour Shift)
• 36 guaranteed regular hours weekly.
• Target start: November 9, 2026, pending facility confirmation.

Coordinate inpatient discharge planning, track referrals and collaborate across clinical services to support safe care transitions.

Requirements

• At least two years of discharge-planning and medical/surgical experience.
• Current unrestricted RN license in a qualifying U.S. state or territory and AHA BLS.
• Complete required Monday orientation and compliance clearance at least seven days beforehand.

Pay and Benefits

Taxable W-2 hourly pay: $28.11–$31.30.
