Assignment Overview

• Albany, Oregon 97321
• 13-week W-2 contract; 1 opening
• Weekday days, 8 a.m.-4:30 p.m.; no weekends.
• Pay illustration uses a 40-hour planning week; 24 hours per week are guaranteed.
• Holiday coverage required.
• Proposed October 28 start, pending facility confirmation.
• Call may be required; standby and callback pay is additional and confirmed before an offer.

What You Bring

• Active Oregon LMSW or CSWA license at submission; LCSW preferred.
• Healthcare-provider BLS and at least one year of relevant experience.
• Valid driver license, insured personal vehicle and ability to travel between patient care locations.
• Hospice experience preferred.
